Add other form fields to the testimonial

I would like the ability to be able to at least display the company name on a line below the job title/tagline field in my testimonial.

Taking this a step further, would it be possible to fully customize what form fields and labels appear on the testimonial and even which lines they appear on?

As an example:

Line 1 [Name] Label Off
Line 2 [Job Title] Label Off

Line 3 [Company] Label Off, [Department] Label Off
Line 4 [Custom field 1]Label ON

Line 5 [Testimonial]

Line 6 [Rating]

Line 7 [Date]

    Widget: Choose different fields under the name

    Option to choose which field appears under the name in video reviews (instead of tagline)

    So by default, what appears under the name in video reviews is the tagline (a.k.a Job Title, Company), but many people who leave a review might not want their review associated with the place their work (they might not feel authorized to do so, etc.) and they might not even want people to know where they work and what role they have, so I would like to have the option, to decide which field appears under the name.

    For example, for my videos I created a field called "Country" so people could enter their country when leaving a review, but unfortunately, I can't display this in the front end because I don't have the option.

    So the workaround I have used is manually adding the country in the "tagline" field.

    But this is of course a hassle and a manual task I would prefer not having to do.

      Upvoted! In my case, we have custom country field in the form. We want to show the country in the tagline additionally

      Thanks for the suggestions and the details.

      Summing up: Add the ability for the account owners to choose which field appears under the name (job title, company, country, or any custom field) to give more flexibility without adding complexity for reviewers.

      Opening this up for discussion and waiting for upvotes.

      Does this feel like a per-widget setting to you, or a global default you'd set once across all widgets?

    What you can do as a workaround is to add your customer’s company name to the tagline field, so it will show in the widgets.
